Primary amyloidosis with renal and myocardial failure.

چکیده
In secondary amyloidosis the kidney is one of the commonest sites in the body for the deposition of amyloid, and renal failure is a common sequel. In primary amyloidosis renal involvement is less common and renal failure exceptional. Only four cases of renal failure due to primary amyloidosis have been described in the literature. The individual patients of Christian (1932), Gerber (1934), and Lindsay (1948) respectively were males whose average age at the time of death was 57 years. The average duration of their illness was three years. The case of Platt and Davson (1950) was in a 60-year-old woman whose illness lasted 18 months. It has been stated (Dahlin, 1950) that death from myocardial involvement occurs in roughly half the cases of systemic primary amyloidosis. The description that follows is concemed with the case of a middle-aged woman who developed progressive renal and myocardial failure due to primary amyloidosis.

Fatal perioperative myocardial infarction in four patients with cardiac amyloidosis.
AMYLOIDOSIS is caused by the accumulation of amyloid fibrils in vital organs, leading to functional disturbances. Approximately 40% of patients with primary amyloidosis have had cardiac amyloidosis. In cardiac amyloidosis, right heart failure is the predominant sign, whereas left heart function is preserved until late in the disease. Because of the cardiac involvement, when patients with cardia...
متن کاملPrimary Hepatic Amyloidosis Presenting as Acute-on-Chronic Liver Failure
Systemic amyloidosis of amyloid light chain associated protein (AL), also called primary amyloidosis, frequently involves the liver, but rarely causes clinically apparent liver disease. The more common presentation is with acute renal failure. Hepatomegaly and mild elevation of alkaline phosphatase are the most common clinical and biochemical findings, respectively. We report a case of systemic...

Background Multi-organ disease with cardiac involvement carries a very poor prognosis in Systemic AL Amyloidosis. The risk of nephrogenic systemic fibrosis is a significant obstacle in assessing cardiac status using CMR in patients with systemic AL amyloidosis who have advanced renal failure. Measurement of myocardial T1 values has been limited until now, due to long breathhold times. We have d...
متن کاملA case of primary systemic amyloidosis with nail dystrophy
We hereby report a 79-year-old Iranian man presenting with nail dystrophy and subsequent development of purpuric and ecchymotic plaques, hemorrhagic bullae, and infiltrated papules on the head, neck and trunk. Histological examination of the gingiva, bone marrow aspiration, and biopsy confirmed the diagnosis of primary systemic amyloidosis. In this case, nail dystrophy was the presenting sign o...

Primary AL amyloidosis is a plasma cell dyscrasia characterized by the deposition of monoclonal immunoglobulin light-chain protein. The latter forms insoluble fibrils with -pleated sheet configuration within a variety of tissues, resulting in severe organ dysfunction and poor outcome. In patients with primary AL amyloidosis, cyclic treatment with melphalan and prednisone improves by 2-fold medi...
